The Pharmaceutical Industry Discovers the Importance of Weight

Roche aims to enter the weight-loss market, which has so far been dominated by Eli Lilly and Novo Nordisk. New study data on the active ingredient enicepatide show significant weight loss and improved blood sugar control in people with diabetes. For Roche, the results represent an important step forward. The key factor now will be how the active ingredient performs in the further development process and in comparison to established competitors.

Roche is looking for a new area of growth

Switzerland is a major hub for the pharmaceutical industry. Roche and Novartis are among the largest pharmaceutical companies based here. At the same time, the focus of growth in the pharmaceutical industry is shifting. One particularly dynamic area is obesity drugs. According to Reuters, the global market could reach annual sales of around $100 billion within the next decade (Reuters, 21.09.2026).

For Roche, the market for obesity drugs is a new business area. The company has primarily grown through cancer drugs and diagnostics. With “Enicepatide,” Roche is now attempting to carve out a niche in a market that has so far been dominated mainly by Novo Nordisk and Eli Lilly (Reuters, 22.09.2026).

Enicepatide was originally developed under the name “CT 388” (FuW, 22.09.2026). The active ingredient mimics two hormones produced naturally by the body, known as GLP-1 and GIP. Simply put, they are designed to curb appetite and thereby reduce body weight. At the same time, they are intended to help control blood sugar. The drug is administered via injection once a week (Reuters, 22.09.2026).

The study data are remarkable

In a Phase II study, Roche investigated enicepatide in adults with type 2 diabetes who were overweight or obese. The results are particularly striking because the drug achieves two goals simultaneously: it helps with weight loss and improves blood sugar levels. At the highest dose, participants lost an average of 15.5 percent of their body weight after 48 weeks (FuW, 22.09.2026).

A significant effect was also observed on blood sugar levels. The so-called HbA1c level, which reflects average blood sugar levels over an extended period, decreased by 2.65 percentage points at the highest dose. The effect was particularly pronounced in patients whose blood sugar was poorly controlled at the start of the study (AWP, 22.09.2026).

For Roche, the weight loss is particularly interesting because people with type 2 diabetes typically lose less weight on these therapies than people without diabetes. According to Roche, a 15.5 percent weight loss in patients with diabetes has, in some other studies, been achieved only after 72 weeks or later (FuW, 22.09.2026).

In addition, the weight curve had not yet reached a clear plateau after 48 weeks. Further weight loss is therefore conceivable. However, future studies will have to show whether this effect persists with longer-term treatment (FuW, 22.09.2026).

Roche Faces Stiff Competition

This brings Roche into a market where two major players are already established. Novo Nordisk has a strong presence in the weight-loss market with Wegovy. Eli Lilly also has successful drugs on the market in the form of Zepbound and Mounjaro. Both companies are also working on additional active ingredients.

The data on enicepatide so far are interesting when compared to these therapies. In terms of weight loss, the effect observed in the Phase II study was at the upper end of what has been seen with comparable drugs. Finanz und Wirtschaft compared the results with Eli Lilly’s tirzepatide. After 48 weeks, enicepatide at the highest dose was in a similar range (FuW, 22.09.2026).

However, such comparisons should be viewed with caution. Enicepatide and tirzepatide were not directly compared in the same study. Differences in participants, study duration, and study design can influence the results. Roche therefore points out that a direct comparison between different studies is only possible to a limited extent (AWP, 22.09.2026).

For Roche, one thing is particularly crucial: The data to date show potential but must be confirmed in larger and longer-term studies. Only then will it be possible to better assess the role enicepatide could play in competition with established therapies.

The decisive test is still ahead

Despite the positive data, enicepatide has not yet been approved. The results to date come from a Phase II trial. In the next stage of development, the drug will be tested in significantly larger groups of patients. Only then will it be possible to better assess whether the results to date are confirmed.

Two Phase III studies on weight loss are already underway. Additional studies on blood sugar control and potential cardiovascular diseases are scheduled to begin in the first half of 2027. In these studies, Roche also plans to investigate whether enicepatide could reduce the risk of events such as heart attacks or strokes (Roche, 22.09.2026).

The safety profile observed so far also provides an initial impression. The most common side effects were mild to moderate gastrointestinal symptoms. Two percent of participants in the enicepatide groups discontinued treatment for this reason (AWP, 22.09.2026).

For Roche, enicepatide could thus become an additional growth opportunity. However, it is not yet possible to estimate how much of this potential will ultimately benefit Roche. Successful Phase III trials, subsequent approval, and its positioning relative to already established therapies would be decisive. Only then will it become clear what role Enicepatide could play in the weight-loss market.
